What caused the air traffic control system to fail? - wrong voltage hahaha?

It is more fun criticizing






The air traffic snafu which affected 55,000 air travelers and cancellation of 300 flights - disruption of 
airline business and less tourism business (vans, tourist guides, hotels were attributed (excusers and escape goats) to:

       1.   Diversion of P12 billion budget for upgrade of Air Traffic  system to cosmetic changes
             at the airport
       2.   Meralco 
       3.   Outdated system

Many see these as flimsy alibis:

      1.  Tugade denied that he ever committed the diversion of budget
      2.  Meralco announced that there was no voltage fluctuation at the area 
      3.  The system is being updated automatically by the vendor.   Besides, it was working well prior
           to alleged power failure

What really caused the system failure?
      The cooling system fan conked out and somebody plugged in the 220 volt system to 380 volt
 Hahaha.    Heads must roll for this incomitance.

      Discipline and strict adherence to procedure is a must for this critical operations.  Back up personnel
back up UPS is a must.   Even tetser for voltage testing?   DOTR and/or CAAP have not provided for that

     Even a big event failure like Chernobyl was attributed by someone who failed to observe the corrrect procedure for the shutdown test.
